Please arrive at least 30 minutes prior to your appointment time. This allows us to process your paperwork and collect any co-payment, deductible, cash deposit, or other payment – ensuring your appointment is spent with the doctor rather than on these administrative matters.
Please be sure to bring with you:
- Your current insurance information and card(s)
- A valid driver’s license or other photo ID
- Any completed patient forms
- Any diagnostic or other test results that may have been ordered or performed by another provider
- A list of your current medications, including dosages
- Payment for services
