Insurance & Payments
Sim Family Clinic participates with most major insurance plans and Medicare. If you have medical insurance, please provide that information to our staff when you check in for your appointment. We will be happy to file claims with your insurance company on your behalf.
Please be advised that different insurance companies may cover different services, and some services you assume to be covered may not actually be covered. While we will gladly assist you in determining whether specific services and/or procedures are covered by your insurance plan, we strongly encourage you to contact your insurance provider for verification.
Please have your insurance card with you at the time of your visit. If we file your claim, you are responsible for the co-pay/deductible/co-insurance at the time of visit. If we are not a participating provider for your insurance, if you are not insured or we do not file with your insurance company, you will be responsible for the entire charge at the time of service.
