What to bring for your appointment

For timely service, please arrive at least 30 minutes prior to appointment time. Please bring your insurance card and information, a list of current medications if any, and payment for services. For new patients that are not insured, we require a $50 cash deposit (no credit cards accepted) at the time of check-in. Additional charges may apply depending on the cost of services provided.
